1️⃣ Definition of Cold Chain

Cold chain is a system of storing, transporting and handling vaccines at recommended temperature to maintain their potency (effectiveness).


Cold chain ensures vaccines remain:

  • Safe

  • Effective

  • Stable

from manufacturer → health center → patient.


2️⃣ Importance of Cold Chain

Vaccines are temperature sensitive biological products.

If temperature is not maintained:

  • Vaccine becomes ineffective

  • Immunity not developed

  • Disease risk increases

  • Wastage of vaccine occurs


Importance in Nursing Practice

  • Ensures effective immunization

  • Prevents vaccine failure

  • Reduces disease spread

  • Maintains vaccine quality


3️⃣ Temperature Requirements

Standard temperature range:

+2°C to +8°C


Vaccines should not:

  • Freeze (damage vaccine)

  • Overheat (reduce potency)

5️⃣ Cold Chain Equipment

A. Storage Equipment

1. Walk-in Cold Room (WIC)

  • Large storage facility

  • Temperature maintained +2°C to +8°C

  • Used at national level

2. Walk-in Freezer (WIF)

  • Stores vaccines requiring freezing

3. Ice Lined Refrigerator (ILR)

Most commonly used in PHC.

Features:

  • Maintains temperature even during power failure

  • Stores vaccines

Temperature:

+2°C to +8°C

4. Deep Freezer

Used for:

  • Ice pack preparation

  • OPV storage

Temperature:

-15°C to -25°C


B. Transport Equipment

1. Cold box

  • Used to transport large quantity vaccines

2. Vaccine carrier

  • Used during immunization sessions

Contains ice packs.

3. Ice packs

Maintain low temperature during transport.


6️⃣ Vaccine Storage Guidelines

General principles

  • Store vaccines in recommended temperature

  • Keep vaccines in original packaging

  • Do not keep food in vaccine refrigerator

  • Do not expose vaccines to sunlight

  • Maintain temperature chart

Top shelf:

  • BCG

  • Measles

  • OPV

Middle shelf:

  • DPT

  • Hepatitis B

  • TT

7️⃣ Vaccine Vial Monitor (VVM)

Small label present on vaccine vial.

Indicates heat exposure.

Colour change indicates vaccine damage.

8️⃣ Open Vial Policy

Break in cold chain means temperature not maintained.

Causes:

  • Power failure

  • Equipment failure

  • Improper storage


Result:

Vaccine potency reduced.

Nurse responsibilities:

  • Maintain temperature log

  • Check thermometer regularly
