GENITOURINARY FISTULA

Genitourinary fistula - Abnormal connection found between genital tract and urinary bladder, is known as genitourinary fistula.


1. Vesicovaginal fistula

In a female, abnormal communication found between urinary bladder and vagina is known as vesicovaginal fistula.


♦️ CAUSES

  • After childbirth such as due to obstructed labour, prolonged labour etc.

  • Trauma

  • Carcinoma of vagina, cervix or urinary bladder.

  • Injury during surgical procedure such as during abdominal hysterectomy etc.


♦️ CLINICAL MANIFESTATIONS

  • Constant dribbling of urine through the vagina is its characteristics feature.


♦️ MANAGEMENT

  • A Catheter drainage of the bladder for some days. It closes the abnormal connection spontaneously.

  • A Surgical management - Surgical repairing of the fistula is done.




2. Ureterovaginal fistula

Abnormal communication found between ureter and vagina, is known as ureterovaginal fistula.


♦️ CAUSES

  • Trauma

  • Surgery in pelvic area

  • Infection

  • Radiotherapy


♦️ CLINICAL MANIFESTATIONS

  • Continuous escape of urine through the vagina.


♦️ MANAGEMENT

  • Correction of fistula by surgical procedure.




3. Urethra vagina fistula

Abnormal communication found between female urethra and vagina, is known as urethrovaginal fistula.


♦️ CAUSES

  • After childbirth such as during prolonged labour, obstructed labour etc.

  • After incontinence surgery.

  • Trauma such as pelvic fractures.

  • Radiotherapy

  • Carcinoma of cervix or vagina etc.


♦️ CLINICAL MANIFESTATIONS

  • Continuous dribbling of the urine through the vagina.


♦️ MANAGEMENT

  • Surgical repairing of the defect is done.




4. Rectovaginal fistula

Abnormal communication found between female rectum and vagina, is known as rectovaginal.


♦️ CAUSES

  • Injury during childbirth such as during obstructed labour.

  • Trauma during surgical procedures in the pelvic area.

  • Carcinoma of vagina, cervix or intestines.

  • Inflammatory bowel disease such as Crohn's disease.

  • Radiotherapy


♦️ CLINICAL MANIFESTATIONS

  • Passage of flatus or stool through the vagina.


♦️ MANAGEMENT Management

  • Surgical repairing of the fistula is done.
